KINDERGARTEN REGISTRATION INFORMATION
Students who turn five before August 31 are eligible for our Kindergarten program.  All incoming students are required to participate in Child Outreach Screening.  Please contact Julie LePain, Child Outreach Coordiantor, at [email protected] or 647-7560 to schedule an appointment.

​If you are an existing parent/guardian and adding a New kindergarten student.
Please click on the Parent Portal Button below to begin the registration process. 
​Once you enter your Portal click on the "More" button and choose "Online Registration".  The system will pre-populate your family demographic information.

​If you are new to North Smithfield Public Schools and completing a FIRST TIME registration.

Before you begin, please gather the following:
  • Household information -- address, phone numbers, proof of residency, and notarized Residency Form
  • Parent information -- work and cell phone numbers, email addresses
  • Student information -- demographic and Birth Certificate
  • Health information -- RI School Physical Form (or acceptable physician form) signed by Healthcare Provider, Immunizations (all entering students must present evidence of immunization with month/day/year for the following immunizations:  Diphtheria, Tetanus, Pertussis, Oral Polio, HIB, Hepatitis, Measles, Mumps, and Rubella, HPV, Tuberculosis, Lead Screening),  Vision Screening and health conditions/medication information
  • Emergency Contact -- phone numbers.
  • Transportation Form​
